I have already e-mailed this to Mint twice, but:

When trying to add a 1st Source Bank account, it asks for 3 things: Answer 3 (I have no idea what this is), Member Number, and Password.

I suspect Answer 3 *might* have been the answer to a security question, BUT I was only ever asked that question if I forgot my password, not just to login.

Now then, last month 1st Source started changing to a new online banking system. A user logs in, and it would say "upgrade to the new banking system." Once you do so, you get a new login screen, and you can no longer use the old login. The one on the home page is the old login, and trying to login there gets the message "this account has been upgraded to the new system - login here instead." I have already upgraded, therefore if that Answer 3 blank was for a security question, I can't get into the old system to see it, and the new system doesn't have a security question at all.

Complicating the issue is the Member Number blank. Under the old system, you had no user name, and the only way to login was to use the last 7 digits of your debit card number. Under the new system, you no longer use that number - instead, you create a username. So which to use for Mint?

Mint may need to change something behind the scenes to allow us to add 1st Source accounts, but if anyone has successfully added a 1st Source Bank account, please let me know how you did it and if you were using the old or new system.

Hmmm...I know I filed a bug for 1st source already.

Can you send me the login url you're currently using to feedback@mint.com? I would like to check it against what we currently have.

Username: Do they let you sign-in with email address, or do you have to do it with a unique username?

For the new system, I just tried - it won't let me log in with an e-mail address, only my username. I'll e-mail you the login urls.
